Notices:

• It is unlawful to keep a child from a parent unless a court order is on file.
• Any person under the age of 18 must have a notarized letter on file to authorize pick-up.
• Persons not listed above can be authorized for pick-up by anyone listed above.
• Every person picking up a child must have photo identification and a copy placed on file.

1. Registration fee paid in full.
2. Complete all necessary information on registration forms.
3. Provide a valid and original birth certificate.
4. Provide a physical and up to date immunization record not OLDER than 12 months, signed and stamped by a physician as required by Illinois State regulations.

1. Deliver and pick-up the child within the times specified ($1.00 per minute late fee).
2. Always leave child with an adult.
3. Keep child home when displaying signs of illness.
4. Arrange to get the child promptly if he/she becomes ill during school hours.
5. Notify the school if the child has a communicable disease.
6. Notify the school of any planned changes of attendance.
7. Attend conferences as scheduled to discuss the child's progress.
8. Participate in meeting the required fundraising goal of $200 per child enrolled or pay the equivalent.
9. Students arriving after 9:00am may not receive breakfast/A.M. snack.
10. Students are asked to arrive prior to 10:00 am.

1. Tuition is due on Monday morning when the child arrives.
2. All programs for subsidy recipients must pay the co-payments in full on the first day of attendance for the month.
3. Tuition not paid at the appropriate time will result in a $5.00 late fee each day per occurrence.
4. Staff will be instructed not to accept the child if the tuition is not paid as expected. Full tuition must be paid before the child is allowed to return.

1. Children attending school for three (3) days is considered a full week.
2. Children are allowed three (3) half week tuition during the school year; after which, full tuition is due.
3. Each child is allowed two (2) full weeks of vacation without penalty for tuition.
4. Children absent frequently will lose placement after a notice from management.
5. Children absent for three (3) or more days require a doctor's note to return to school.

In signing this Parent Contract, I am stating that I have read, understand, and agree to adhere to all the terms listed above and those outlined in the Parent Handbook including the center's discipline and behavior policy. I acknowledge the receipt of DCFS Licensing Standards. I also acknowledge that a member of management was available to answer my questions. A violation of this agreement can result in the dismissal of your child without notice.

Dear Parents/Guardians,

The first 5 years of life are very important to your child because this time sets the stage for success in school and later in life. During infancy and early childhood, many experiences should be gained, and many skills learned. It is important to ensure that each child's development is proceeding without problems during this period. Therefore, we are interested in helping you to follow your child's growth and development.

Screening and Monitoring (Assessment)

Our teaching staf are trained to administer the Ages and Stages Questionnaires. These questionnaires help the staff plan age and developmentally appropriate activities/experiences for every child enrolled and program improvements. You can help us by completing the questionnaire that will be given to you by your child's teacher at 2-, 4-, or 6-month intervals. Answer the questions to the best of your knowledge and return to your child's teacher.

In addition to these screenings, our staff complete continuous observations and informal assessments. When concerns arise, further observation, assessment, or referral to/from our school nurse and mental health specialist may be necessary for further guidance. You will be notified of any concerns and results from our support staff's review.

Communicating Results of Screening and Assessments

Results of the Ages and Stages screening will be sent for your review regarding your child's developmental and social-emotional acquisition. If there are any concerns noted by the teacher or parent, we will contact you directly to provide further assistance in addressing those areas of concern.

Assessment is an on-going process and occurs throughout each day as the children play and interact with their peers and environment. We communicate your child's development, interests, and needs through monthly "As I Grow" reports, Parent Teacher Conferences at least twice a year, student portfolios, Daily Sheets, Progress Reports, and Report Cards (respectively, for each age group).

Please keep in mind that all information involving your child will remain confidential. At any time, you can ask about your child's development and express concerns and achievements.

We ask that you commit to working with us as we dedicate ourselves to "Shaping Young Minds".

I also authorize support staff (school nurse, mental health specialist, etc.) to conduct further observations and evaluations, as necessary.

The first goal of child guidance is to help children learn social skills that will enable them to get along with other people. The second aim is to maintain classrooms in which each child can feel secure and comfortable while learning. Methods used by teachers will facilitate the children's development of self-discipline and socialization skills.

For infants and toddlers:

1. Meet babies' needs for love and care and build a trusting relationship with them.
2. Prepare the play space thoughtfully and make it childproof.
3. Accept children's feelings and provide outlets for them.
4. Refocus toddlers' attention before inappropriate behavior occurs.
5. State directions clearly and simply.
6. Be calm and consistent.
7. Allow children time to adjust to transitions.

For preschool children:

1. Arrange classrooms that are comfortable, interesting and encourage children's self-direction.
2. Help children express their emotions verbally and through art and play.
3. Allow children to resolve their own conflicts when possible.

4. Model and teach children strategies for solving interpersonal conflicts, like negotiation, comprise and empathy.

5. Help children learn to anticipate logical consequences of their behaviors.

6. Involve children in cooperative projects.

7. Assist children in setting clear, consistent, fair limits for classroom behavior.

It is not acceptable for adults to administer negative discipline:

1. Inflicting physical pain (suspected child abuse will be reported to the child protection agency);
2. Name-calling, shouting, threatening, ridiculing, etc.

3. Depriving a child of any service-transportation, field trips (unless there is a safety issue and a chaperone is not available for attendance), food, daily attendance;

4. Isolation;
5. Sending a child out of their regular setting;
6. Imposing cumulative or delayed consequences.

If staff is concerned about a child's emotional well-being based on observations of his/her behavior, they will follow this procedure:

1. Discuss the behavior with Management.
   

2. Consult with our mental health professional after she has observed the children in the classroom. 

3. If indicated, initiate a conversation with the parent to ask permission to have the child evaluated. (This must be conducted in the family's primary language and should follow the form of any discussion of special services. It is not the child's behavior that is the first concern, but the reason behind the behavior. For example, the child seems upset, unhappy, angry, depressed, frightened, or anxious. In no case should it be implied that the child or the parent is bad or abnormal. The staff offers to help the child and parent with any of the services available through the program, with the understanding that this is the function of the program).

4. If the parents refuse permission to evaluate the child, or the help of other services, remain in contact with them and continue to offer them support. Ask the mental health professional and grantee managers for recommendations for helping the child in the classroom. Hire extra staff if necessary and if possible. If children have toileting accidents, or are not well potty-trained, they are not in any way penalized. Children are placed in the room with their age group, regardless of potty status. Teachers may ask the cooperation of the parents in beginning potty training, but no special measures will be imposed upon the child or parents.

Suspension, Expulsion, and Exclusion

P.A. 100-0105 prohibits early care and education (ECE) providers from expelling young children (ages 0-5) from their program because of the child's behavior. We must take documented steps to address the child's behavioral and other needs to keep the child in care, and if ultimately necessary, we will work with the family on a 'planned transition' to a more appropriate setting; this 'planned transition' process is not considered an expulsion. Our center's goal is to limit the use of suspension, expulsion and other exclusionary methods and measures.

• Documentation.
  When a child shows consistently challenging behaviors, we must document our efforts to address the child's needs and keep him or her in the program, including but not limited to:

  • Initial observations of challenging behaviors

  • Communication with the family, family's input in any intervention, and family consent for or refusal of services

  • Plan for addressing issues with the child, including resources/services/interventions contacted and used

  • If and how the child is transitioned out of program (in situations where this is determined to be most appropriate)

• What if the child is an immediate threat to the safety of him/herself or others?
  In the case of serious safety threat, the child may be temporarily removed from the group setting. We will document the incident, and work with the family and available resources to return the child to a group setting as soon as possible. If our professional judgment is to transition the child out of the program, the process will be started.
  

• What is a planned transition?
  If we have tried many times to use all available resources and have come to a decision that keeping the child in the program is not in the best interest of the child or the child's peers, the center will work with the family to transition the child out of the program. Our center will work with families to find a more appropriate setting by making referrals to other programs.

I give permission to Under CARREY'S Care Corp. to take whatever emergency measures (i.e. first aid, disaster evacuation, medical services, etc.) that are judged necessary for the care and protection of my child while under the supervision of the center.

In case of medical emergency, I understand that my child will be transported to the nearest hospital by the local emergency unit for treatment if the local emergency resource (police, fire, or ambulance) deems it necessary. The child will be transported at the expense of the parent, guardian, or insurance provider.

It is understood that in some medical situations, the staff will need to contact the local emergency resource before the parent, child's physician and/or any other adult acting on the parent's behalf.
